If you have access to a printer, you can speed up your check-in by printing and filling out forms ahead of time. Most insurances require updated forms if you have not been a patient in the past year.
Please bring copies of medical records, X-rays, MRIs, nerve studies, etc. It will help if you bring the films (or CD, USB drive) themselves, not just the reports. If you need to request the films from another facility, allow some time for this.
Please arrive 15 minutes early to take care of registration. Remember to bring your identification and insurance information, and contact your insurance company to determine your benefits and inform us of any special requirements. Many insurance companies (including those related to automobile or worker's compensation) require that we receive a referral or authorization letter prior to seeing you as a patient.
